Start with your own question

Open the book peptide clinic notebook

Write down the question you came with, the exact medicine name you saw, and every word that still feels unclear.

You have six tabs open and every page uses different words

Stop trying to compare every page at once. Write down the one thing you want help with, the exact medicine each page mentions, and who would be responsible for your care.

Three notes make the rest easier

These notes organize what you found. They do not decide whether a treatment is right for you.

Name the goal

What did you hope a visit might help you understand? Keep it in your own words.

Name the medicine

Copy the generic name—the active medicine’s name rather than a brand name. For example, semaglutide is a generic name and Ozempic is a brand. If no exact name appears, write that down too.

Name the people

Leave space for the name and professional title of the person responsible for your care, plus the pharmacy’s name. Later pages show where to check both.

Write only what you know so far

Nothing here is sent or saved. Copy your notes somewhere private before leaving the page.
